[Plugin: Drain Hole] Missing an “upload” field

Hi,

The screenshot here (http://urbangiraffe.com/images/plugin/drain-hole/add-file.png) has a clearly marked upload field. However, it is missing from my installation (http://www.ubuntu-pics.de/bild/6467/screenshot_98_BxEw06.png)

Did I do something wrong?
