[Plugin: Email Before Download] Styling the form

What you can see there is just the styles that are applied to the div html element that surrounds the form.

Anyway, we are planning to provide the html and css sample somewhere in the FAQ, if not options in the plugin admin itself soon.

As for your second question about getting inline download link to show. Please make sure your have installed and activated the required plugins. Email Before Download is working with the latest versions of them:
Contact Form 7 Version 3.1.2
Wordpress Download Monitor Version 3.3.5.6

Also check if you can send emails from your server, this has to be configured correctly in order to make CF7 work, which is key to our plugin.
