[Plugin: Fast Secure Contact Form] can't make captcha fields show up

i loaded the plugin from the wordpress dashboard. contact page looks right except no captcha. i checked the folder permissions and they are 755 as suggested. (note user/group and everyone write boxes are not checked. all other boxes are checked.). akismet is not enabled. my web host is lunar pages and they are using PHP 5. i checked the settings and i even checked off the box regarding hide/unhide. nothing seems to work. can anyone help? thank you.
