[Plugin: Fast Secure Contact Form] Form disappear … why !?!

Hi all,
I’ve a problem with Fast secure contact form.
It’s very strange because until now, it work perfectly !

The form doesn’t appear … If I deactivate and then activate, I can see he form.
But after some hours, it disappear !!

I see like this :
“I’ll be happy to provide, all the information you need.
I am willing to travel anywhere in the world to capture your special moment.
[si-contact-form form=’1′]”
