[Plugin: Fast Secure Contact Form] input names and checkbox-multiple

Hi Mike, I have a couple of questions.

1. When I define an extra field, I want to set the input tag’s name attribute (to do some processing on the back end). I tried adding name=”my_name_to_use” in the “Attributes” field but then I get two name attributes output in the this HTML input tag, i.e.: name=”si_contact_ex_field3″ name=”my_name_to_use”

2. I want to use checkbox-multiple. So I went about adding an extra field for each check box. But I need to have them all have the same “name” attribute to behave as a multi-select. I don’t see how to define that.
