[Plugin: Formidable Forms] Form not displaying

I completed a form, added it to a page, and it isn’t displaying correctly. All that shows, whether I’m logged in as Admin or a normal user is an “Edit Form” link and a save button. And the Edit Form link doesn’t do anything.
