[Plugin: Formstack Online Forms] Appearance On Word Press Page

The appearance of my Form does not look as good when displayed on Word Press.
http://spacecoastcrm.com/contact-us-2/ . Too much vertical white space between fields. Also, the field labels for First Name and Last Name have too much white space above them. Same is true to for City, State, and Zip Code. I am novice with both WordPress and Formstack so please be very explicit with any suggestions. Thank you in advance!!!
