Plugin help (Need similar plugin)

Hello everyone, I need similar plugin like this one:

So,I need to create subjects for school and students can choose one favorite and submit it. New tab will open and they can put their email,phone number, etc and submit that to us. I would like that I can change style and color of layout,submit button,etc. Thanks in advance
