Plugin: Hidden Login & My own template

Can anyone tell me what I need to have in my template to make this plugin work? I presumed the plugin would insert the relevant code but it is not working.

Also the admin toolbar at the top of the page when viewing the site while logged in has a gap where the toolbar should be but there’s no bar
