[Plugin: Jazzy Forms] Form not functioning

I’m trying to do a simple calculation. The user enters length and width to get the area.

Do you know of any plug-ins that conflict with this plug-in?

I contacted the template developer and he couldn’t understand why it wasn’t working. I’m looking to you to see if there’s something conflicting? Does it conflict with Contact 7 Forms?
