[Plugin: Leadin] Not Receiving Leads from forms

My company is trying to use this plug-in to capture the path of each individual lead as well as get initial contact information from each person. When a form is filled out we are still getting the normal notifications and it is working the same prior to the plug-in, but it is not capturing the contact information or integrating property with Gravity Forms.
Unsure of the cause since it’s a highly rated plug-in, but can’t figure out how to make it work correctly.
