[Plugin: LeagueManager] Missing button at add team page

Hi,
I’ve got a problem when I try to add a team. I’m missing the button to actually add the team after I fill in its name, logo, etc. Also the top admin bar disappears at the page.
I don’t have the problem on other pages like add season or match.

Could anyone help me please?
