[Plugin: MailChimp List Subscribe Form] Fatal Error Message

I’m getting a Fatal Error message with this plug in.

It’s a text message within a styled box, with big red lettering. There is a link that says “show” that I would assume tell me what the issue is, but it goes nowhere. (I’ve seen these types of “Fatal Error” messages in WordPress before.)

I see this on all three pages for the plug in, but the message is not displayed elsewhere.

I am running the MailChimp Setup plugin, but I deactivated it. The styling we have on the form we are using now is just terrible (http://zamblings.com in the right sidebar).

Also note, you mention a Github Issue Tracker at https://github.com/yikesinc/easy-mailchimp-integration (in the plugin notes on the Manage Lists page) bu that link is invalid.
