[Plugin: MailChimp List Subscribe Form] Feature Request: Include Preferred Format

Dear MailChimp,

I really like your plugin, but there is an important feature missing: The sign-up form does not allow the subscriber to select a preferred email format (HTML, Text, Mobile). This feature is available through the default sign-up form, and through the embedded code generated on MailChimp.com, but not through this plugin!

Here’s how the plugin looks on a website I’m building for a client:

Here is the default sign-up form (with preferred format), first on it’s own, and then embedded into a Facebook page:

I’d like the sign-up form to include the preferred format no matter where it appears. Can you please add this feature to the WordPress plugin?
