Version 3.0 of the plug-in does work fine with WP 3.0.1 and the newer Twenty Ten theme; however, it’s not going to stop a human manually filling out your form and answering the math question to put in spam. At best these math protection plug-ins will stop some of the automated spambots.

Actually MarkRH, akismet or any other software known to man will NOT stop what you basically describe.
Akismet MAY be able to stop a cut and paste human manual spammer. But not the true manual spammer.
Spambots account for like 99% of Comment Spam. I found that by taking a little time on the settings page, I could make MCSP more effective by making it appear unique. Spambots are looking for the usual.
I reformatted the challenge or question, and change the vars on the settings page – and have so far blocked 100% of comment spam on 24 sites. It is the ONLY Comment spam protection I use.

PS I like that MCSP has a low overhead also.
