Plugin not working

Plugin appears not to work on a local site using Genesis framework with Attitude child theme. Deactivated all plugins.
The image at: https://wpovernight.com/faq-category/bulk-order-form/
shows a section in a red box that is not showing on my site.
Any help would be appreciated as the plugin looks promising.Regards Michael
