In Download Monitor, have you specified a URL (i.e. starting with “http://”) to your file or a location on your internal server (i.e. starting with “/users/”)?
Email Before Download currently support URLs in the “Edit File URL” box within Download Monitor, which is simple to populate if find that you have specified an internal server location.
Feel free click “Edit” on your Download Monitor download and paste what you have in the “Edit File URL” for further review if needed (you can obfuscate directories for security when posting to the forum if desired).

The url specified in Download Monitor is
/homepages/13/d332028505/htdocs/content/xyz.zip
This configuration works fine when using the download monitor link (http://mysite.com/wp-content/plugins/download-monitor/download.php?id=12)
but the the links Email Before Download returns of the same file do not work
this is the link EBD returns with “Hide/Mask Real Url” on (does not work)
http://mysite.com/wp-content/plugins/email-before-download/download.php?dl=199a5fd1dd67dfa47214d42388210b2d
and this is the link EBD returns when I try it with “Hide/Mask Real Url” off (does not work for obvious reasons)
http://mysite.com/homepages/13/d332028505/htdocs/content/xyz.zip

You should be able to change your server path:
/homepages/13/d332028505/htdocs/content/xyz.zip
To an actual URL, something starting with:
http://mysite.com/…
