Plugin Unresponsive to screen size changes

    Having issues with the responsiveness of the form.

    When displaying on for example a mobile device or a tablet the layout of the form is altered and things are displayed vertically rather than horizontally.

    I have tested with different themes also to see if the theme was causing the issue however the result is the same
