    You need to publish the page and then view it in order for the form to display.

    Did that many times. Does not work. I put shortcode into html view.

    It might be a plugin or something in your theme causing problems. Deactivate all other plugins and switch to the default Twenty Elven theme.

    Decided to use another plugin that works with my theme.
