[Plugin: WordPress Form Manager] Form not working at all.

I’m using WP 3.4.1 and have installed the plug-in, but when I go to create a form and edit it, the only thing that occurs is that the edit link turns to “hide”. Nothing expands and I can not edit or create any form fields.

This works on another site of mine using the the same WP version.

I have disabled other plug-in’s, but I still can not get it to work. Any suggestions? It would be a shame not to use this one since it’s an excellent plug-in.
