[Plugin: WordPress Form Manager] How to re-use a form

Hi, this is a great plug-in. It will made our life easier.
I created a long form in a Blog but now i want to change the URL so I need to migrate the form too, What are the files that I need to migrate in order to re-use the from I already done?
