I received the following fatal error when attempting to activate the plugin:

Warning: require_once(admin/admin_functions.php) [function.require-once]: failed to open stream: No such file or directory in….

There is a file called admin_functions.php in the admin folder as seen here in the actual repo:

http://plugins.svn.ww.wp.xz.cn/wordpress-simple-survey/trunk/admin/

You must have gotten a corrupt download.
