[plugin wp-crm] Name field hidden

Hi all,

i created a new form and i put it in an example page called CRM.

I’ve no problem to include the form but the input field of user name is hidden.

Only if i’m logged in into wordpress show me the field (not editable).

Anyone know the trick ??
