[Plugin: WP Document Revisions] Documents 404 Not Found

I set it up months ago, and I think I remember everything working back then, but I just tried accessing the documents again and get 404 Not Found errors, but not even the theme’s 404 – the server’s 404. Yikes!

I even re-saved Permalinks and tried renaming the pdf files via SFTP to match the URL’s filename and the last thing I tried was re-uploading (version 2) of both and same error – no improvements or differences than version 1 of the documents.
