[Plugin: WP Super Popup] Plugin not working

I have WP Super Popup installed on http://www.decisivedesign.com/oilinfo/ using wordpress 3.3.1 and it is not working.

In the admin, the live preview shows a 404 page, and on the actual site it doesn’t pop up at all.

What gives? Please advise.
