Pop Ups

I’m using the Codium Extend theme and when I navigate to a link to a pdf on my server I get a Survey Monkey pop up. It doesn’t happen all the time or on all browsers. At my office it happened on IE8 and at home Firefox 15.0.1. Pop up disabled or not I didn’t put the pop up there and my host says they didn’t either. They tried using Chrome and IE9 and couldn’t duplicate it. The pdf in question is one I created. So could this be a WordPress thing or is it the theme pack I’m using?

Navigate to the City Information drop down on the menu bar and select Avondale/Avondale Home Statistics

Like I said though it doesn’t happen all the time and I know that I’m not hallucinating
