Popout Page

On a page, I would like to have a link that opens a pop up page (is that called a modal?) using data from the page.

If the page has a field with the number 1, then I would like the pop out form to query some data from the db using the 1 from the field. I can write the queries, but I don’t know how to get the pop out page to come up.
