Populate a Text Area Dynamically

Looking for a method to Populate a “TextArea” Dynamically at the end of a form

Have 50 questions as “YES” or “NO”

If they answer “YES” a “Conditional Fields” “TextArea” shows below the question to allow them to give a required answer.

Looking for method to ADD all the “Yes” answers to a single “TextArea” at the end of the Form before submitting.

Any suggestions, I’m stumped.
