Possible htaccess malware attack

Hi there,
I’m currently trying to figure out how some malware has appeared on one of the websites I manage. The malware has been injected into the site, and every page, post and media file has code in it that brings popup ads up whenever you click anywhere on the site. I have a feeling that the htaccess file has been hacked, but looking at it, I’m really not sure where to start in figuring out which bits are malicious, and which bits are safe.
