Hi

After submit i need send a post like this field with the data from tohe form

Now i use this but that doesn't work

I get in the $data field this information:

I'm realy a beginner with Php so i have no clue what i can do the get fill the $data field with the right content like above?
Is there anybody who can help me a little,
