I hope you are doing well. And thanks for the screenshots; I can see the form is loaded in some kind of pop-up element in your site. We have encountered some issues in the past where a form loaded through a pop-up may have issues due to the code needed to display such pop-up.
Can you please check if the form loads fine when added directly into a page or post?
Are you using Hustle, another plugin from WPMU DEV, to display that pop-up?
Can you please try the following? Edit your form, and go to the “Behavior” section of the form editor page. There, at the bottom, you should see the option to “Load form using AJAX”. Change the value of that setting, clear the cache of your site and try again. Please let us know if the form loads fine after that.

thanks for answering this fast !
Changing the option “Load form using AJAX” helped and solved the problem.
But now the Captcha isn´t showing in the pop up form and the form isn´t be saved when u send it 🙁
The form loads fine when added directly into the page and everything works.
I think this option is a good alternative to the pop up so i will use this.
Btw is there a way to configure the layout of the post image (it´s a great deal too big)?
I´m not using another plugin from WPMU DEV (i think) to display that pop-up, just using Elementor Pro.

I’m afraid, the issue is more related to compatibility. There is already a bug report that our team is already working with displaying the captcha.
Since there are multiple issues, a quick solution would be to consider using the following popup plugin which should work out of the box:

Could you please check whether the above helps in such use cases?

But i will take the solution add the form directly to the page and remove the option with the post image it´s a great deal too big
