Please share a report ID with me. This can be done from LSCWP => Toolbox => Report and click on “Send to Litespeed” and get the ID generated.
Also please try to isolate what action is triggering the 404 errors: is it happening random? is it happening after updating a post/page? is it happening to all?

when you hit 404 , how did you fix it ?
and may I know what is your access level to server? is this a shared hosting ? or you have root access ?
By updating Permalinks, without making any changes.

yeah , it’s kind of wordpress core issue , it happens on sites that doesn’t have LiteSpeed , not exactly related to our plugin or any plugin for that matter , you can find a lot of such post in google search about 404 error and fix by flush permalink.
I don’t really have a good solution to this , but off handed , I feel maybe programmatically flush the permalink by system cron job or something , that should automatically fixes the 404 by flush it again.
@greencode try Purge All after Permalink changes, see if this fixes the issue

no , I mean , the issue happens on any kind of site , like site that is not using LiteSpeed , so it’s not LiteSpeed-related issue
