Same issue here I am using latest wordpress 4.9.6 with the following plugins:
Ninja Forms
Version 3.3.4
Ninja Forms – Conditional Logic
Version 3.0.10
Ninja Forms – File Uploads
Version 3.0.5
Ninja Forms – Layout & Styles
Version 3.0.12
Ninja Forms – Multi-Part Forms
Version 3.0.9
Any clue on what is causing this, previous versions were working fine with my setup and all plugins activated.

Can you both confirm that any caches on your sites have been cleared?
I’ve seen this issue a few times now, and it appears to be related to some of our javascript files not getting properly updated due to cached data.
I am also having a problem with the forms displaying. No other work has been done on the site. I did purge all my cache. And disabled/reenabled plugins. I’m on the latest version of WP.

On our end W3TC was caching the scripts and once I cleared the cache of those specific objects (CDN Purge) it worked fine. Thanks guys 🙂
this has nothing to do with what the guy is talking about. It has to do with minification.
It has to do with the millions of sites that use W3 Total cache
Turn off inline JS minification under “Minify” “HTML & XML” “HTML minify settings:”

But if I disable Minify option Ninja Form works well.
Please fix it, because I want still use Minify option to speedup mu page.

We got a site for our airfield. Now if there is a prolemm we use the ninja forms. But someone told me I cannot upload anything. So I looked and saw that the upload button was dissapeared. Is this happend by pluggin upgrade?
We desperate need it. we are a simple small airfield.
