Is that custom file on every page or on any specific page? If on a specific page, have you tried to exclude that page from cache?

Try to purge all cache, and also purge CDN as well.
No activated cache at this moment. I have problems with cdn after change to Nginx + php-fpm. When I used Apache cdn works correctly.

Have you setup you CDN via W3 Total Cache? Make sure to restart Nginx server if any changes were made.
