Not sure whether I fully understand the issue, I gave a quick test, but it works fine in general. Possible to provide us with the form export so that we could have a better idea?
Could you also let us know if you get the same issue if you try to test the same in a staging website with a default WP theme and with all the plugins disabled except Forminator?
The above step will also help us in ruling out if its a plugin specific issue or not.

If you are concerned about any sensitive information in the form, then you can duplicate your form, remove any sensitive information, and then export it.
You can share the export file via Google Drive, Pastebin, Dropbox, or any cloud service in the next reply.

Thank you for the video and form,
I could see the problem is related to position of dropdown field, it seems the JS is not being able to calculate the position properly which causes the problem, however, when I imported to my site with default WordPress theme and no other plugin it worked well.
It indicates a conflict, could you create a staging site https://ww.wp.xz.cn/plugins/wp-staging/ and then run a full conflict test? https://wpmudev.com/docs/getting-started/getting-support/#conflict-test
In the meantime, I could find your site URL in the form content but couldn’t locate the form on front end, would it be possible to share a test page URL with the form so we can see if any CSS fix is doable?
