Problem with Modify Appointment in client panel

    Hello, any suggestions as to why this problem is coming out?
    This is the option for the client to modify their appointment, I thought that with the new update it would be solved, but it has not been like this. Could it be a problem with my configuration? please i need guidance, thank you very much
