Problem with Registration form

Hello!

I was working on my default registration form, to be exact i was making another custom field, then i lost my internet connection (ISP fail). After getting back on-line i spotted that my registration form is crushed (every field was gone). So i’ve added some regular field, which was ok, but then i tried to add some custom field and i got crush every time i tried it. I’ve cleared temp, cache and delete old registration form. But that doesn’t work :(. Please help!
