Problem with reservation registration

Dear Sirs,
I have just installed and configured your plugin. Everything works fine except I cannot finalize reservation. Below is the scenario:
1.Service selection works fine, then
2.Time period selection works fine
3.After clicking Next system displays information as presented empty page with number 0 (zero) presented in the upper left corner. Reservation is not registered.Any idea what could be a reason for that?

Dear Sirs, Turned off all plugins except yours and situation did not change at all. Cannot change theme, as I am using DIvi from Elegant Themes and I am affraid I’ll loose my data.
I did not apply debug procedure as I don’t have enough experitise in the area.
Is there any other way to diagnoze the source of the problem?
