Problem with themes

The first thing you might try is under the ‘Survey Styling Options’ tab, select ‘No’ for ‘Use included css?’ This will allow the theme to handle forms however it handles forms.

Since that didn’t work, can you re-enable the use of the included css (Survey Styling Options -> Use included css? -> yes) and I’ll take another look at the link you gave me.

Your theme has some css that is interfering with the display of the survey form.

Here’s a css snippet that you can use that should get you sorted:

.answer-survey label {
  width: auto;
  float: none;
}

Your theme should have somewhere for custom css
