Problems with file upload

I use PPOM Pro and added the file upload function. The customer has to upload a passport photo before the product can be put in the basket. On most orders it works well, but it seems that there are a few orders, where the file isnt uploaded in the uploads/ppom_files/confirmed…folder. Also, there is no thumbnail in der order. Allowed files are jpeg, jpg and tif. Any suggestions?
