Please try adding the following CSS code to Custom CSS option of your form theme:

width: 150px !important;
margin: 5px !important;
}
div[wdid="11"] {
margin-top: 20px;
}
Make sure to clear the cache of your web browser before checking the changes. Let us know if it works.
