Problems with using a name capture form on wordpress

my guess is that somewhere you may have accidentally put in a / or closed an element twice. If you know how to code go into the code and find that big of code and then maybe black it out (make inactive, idk the name of it). If there are any repercussions bring it back, if not its fixed!
