After constructing my survey with five questions, the survey status is “Published.” However, when I enter the URL (machiasdike.com), only the first question appears, without the response selections. How can I publish the entire survey?

We checked and couldn’t find a survey in your provided website. As far as we understand, the plugin is activated, but the shortcode is not inserted into the post/page.
Your reply provided what I needed to display my survey. The survey pages, however, have a dark background and a font so small that it cannot be read when the display is zoomed to show an entire page on the screen. How can I reformat the display?
Simply increase the value to make the text larger.
After making all changes, click the Save button (either at the top or bottom of the page). If you’re using a caching plugin, make sure to clear the cache to see the updates on the front end.
