First of all thank you for your great plugin.

But unfortunately QR code does not showed up in my login page. I have tested in 3 separate installation (local server also in live server) but same result. It just show a broken icon (normally shows this icon when we have invalid src in image tag). Any clue?

So i figured out.

Your ajax hashUrl is using hard coded url part which only work if pretty permalink is enabled.

as advice:
You can first check is pretty permalink enabled or not then ad query string.
