Thanks for the this great plugin.

I have a couple of queries;
1. How do I get rid of the name “souvik paul” in white on the top left of this webpage: http://www.bruisedpassport.com? I can’t figure out from where this is picking up?
2. I did the mailchimp settings but still can’t get the email field they way you have in your screenshots. What have I done wrong?
