Questions Pages issue

Hi there,
Please be informed that I found an issue on “Questions” tab on QSM admin panel. During set-up quizzes after reload of the configuration page the questions are gone. It’s going to be messy.

Last time when I’ve set up quiz without any problems it was on 19th of January 2021. Today when I’m adding questions and/or pages (about 10 questions) it looks great until I refresh page (or simply go to the next config tab and go back). The Questions tab looks empty or eventually one question is visible. The “question/page was saved” message showing at the top as expected right after create a new question.

Also this issue breaks currently saved quizzes, and I had to rollback from database to fix previously created quiz twice.
