Radio button response not saved

I don’t understand what you mean. The plugin stores all answers regardless the question type.

From my end it looks OK, I’m afraid I have no way to know what could be wrong on the back-end. There are no visible errors.

Possible causes:

1. Your site source code is “optimized” all on one line. I don’t see jquery.js anywhere too. Switch off whatever source oprimizer you are using, the do not work and the bytes they save are not worth it.

2. There is one Javascript error from another script which might be causing it:

There is still some Facebook pixel error but I have no idea whether it might be causing the problem:

When I look at the Ajax request it looks fine. You may need to check the server error log and/or disable plugins to figure it out. I can’t understand what’s wrong from here.
