Rating forms missing after last upgrade

    I’m getting a problem after the last upgrade. When I go to the EntryWizard Rating Form Management page in the WordPress backend none of my rating forms are visible and I can’t create any new ones. My Rating Schemes are still there and everything else seems to be OK it’s just that all of my Rating Forms are missing.

    If I try to edit the WordPress page that contains the link to one of my rating forms I get a critical error and get an email from WordPress with the following in it:

    Apologies, I missed seeing this earlier. And I’m afraid the above just leads to more questions:

    What does your Rating Form Management page look like? Do you see any list of “hidden” rating forms? Or a “Bulk Actions” drop-down with a “Hide selected rating forms” item on it?

    In the Entry Wizard Permissions page, does your username have permission to Assign All Layouts and Edit Any Rating Form? If not, try giving yourself those permissions.

    When you say you “can’t create any new ones”, what happens when you click the “Add a New” button? Or is the button not visible?

    Do you see any other unexpected changes?
