Redirect old pages

Hello all,
i get a problem in last days.
I have page URL in such way:
http://domain.com/year/month/day/number/
And i want to have
http://domain.com/year/month/day/postname/
how-to redirect old pages, because if i change, old links still exists in google, yahoo and etc. So what?
