You can use the redirect log to see what is looping and the redirect behind it.

hi john, thanks so much for your response, will it still show in the log if a loop has occured? and what if it has by another plugin? or something caused by litespeed / cloudflare? will your plugin still see it?
If Redirection is causing it then you will see an entry each time it is requested. If something else is causing it then no, you won’t see an entry, and you will need to figure out the cause yourself.

i checked the log and looked for redirections that happened around that time it occurred and theres nothing that matches, they all look valid.
on the 28th we had the website go down at roughly 5am where all traffic going to the main url / was being pushed to a utm path causing a loop, for all users the website was unavailable .. i didnt do other testing my priority was to get it fixed and operational

is there something we can add to .htaccess to run redirection logging to see what else is causing it?
No. If it is outside of Redirection then there is nothing that Redirection can do.
I would disable everything and then gradually re-enable things.
